Fiscal Sponsorship Forms
Forms for Fiscal Sponsorship projects are now online to make them easier for you to use and speed up our processing time. The forms are securely encrypted.
Accounts Payable Form
The AP form is used to submit a payment from the fund. As a Project of ACF you can submit invoices and expense reimbursements to be paid directly from the Fund. ACF uses the information submitted in the AP form to process payments in accordance with IRS regulations. Please review the AP instructions page before submitting an AP request.
Event/Fundraising Forms
As a project of the Austin Community Foundation is it imperative you provide ACF with information about your fundraising activity. You must notify ACF at least 1 month prior to the event/campaign or when you plan to start accepting sponsorships.
Use the check deposit form to inform ACF staff of incoming checks to your fund. Please indicate on the form if the check is 100% donation, services were received by the donor, sale of object, or 100% non-gift. These gifts will be reflected in the fund 2-3 days after they are received in the office.
Use the fundraising application to inform ACF or upcoming fundraising activity that is not event specific. Examples include an individual giving campaign, membership drive, etc…
Use the event notification form when you are planning an event. This form is also used to capture information about sponsorships, ticket sales, fair market value, and to create custom donation forms. Please provide for a full week turnaround time on custom donation forms.
Use the In-kind gift form to acknowledge receipt of in-kind gifts. ACF will take the information provided and generate an acknowledgment letter for the donors use. Do not include a dollar value when complete the form.
Use the custom donation form to request that ACF create a custom donation form to embed on your project's website. This custom donation form will take on all style and security characteristics of the project's website. Please allow a one-week turnaround.
Reporting Forms
A new requirement of Fiscally Sponsored Projects is to submit two reports annually that inform the Foundation of your impact and fundraising activity. This is a necessary requirement since your Project is operating as an extension of the Foundation's work in the community.

In addition to the annual report, Projects are required to submit any grant report to ACF prior to submitting it to the granting organization.
